Hot Lunch Order Form
        Instructions      
             
     1.  Please print a copy of this form as well as the monthly menu(s) from which you would like to
     order. Please  return a separate menu for each child who is ordering lunch.  
     2.  Place the student's name at the top of the menu and circle/highlight/mark the days of the
     month that you wish to order.          
     3.  Count the number of days ordered from the menu(s) and place the total in the appropriate
     space below            
     4.  Multiply the "Number of Meals Ordered" by the appropriate "Cost per Meal" and place
     that amount on the grade level total line.        
     5.  Please add the amounts from the "PreK - Grade 3 Total" and the "Grade 4 - Grade 8 Total"
     and place the answer on the "Hot Lunch Order Grand Total" line.    
     6.  Please remit this form, all menus, and a check made out to "Heavenly Scent" equal to the 
     amount listed on the "Hot Lunch Order Grand Total" line to the All Saints' Episcopal Day School 
    front office.
